New Patients will receive a packet of
information in the mail with an appointment card verifying the time and location of their appointment,
a Medical Records Release form, plus instructions and paperwork for the first visit.
We also include a map with driving directions to our office.
Most New Patient visits require a referral from a primary care or other provider. We will need chart notes and copies of lab work from your other doctor(s) related to your endocrine condition as well as a list of your current medications. Click

Your first visit will usually last about an hour. Please be aware of the following policies regarding New Patient appointments:
You will be asked to confirm your appointment at least
72 hours in advance of your appointment time. Unconfirmed appointments risk cancellation and rescheduling of the allotted time to someone on our waiting list. We only allow one rescheduling of a missed New Patient appointment.
We ask that you bring the following items to
each appointment with us: An up-to-date listing of all
your medications including those you purchase over-the-counter,
any insurance co-payment and your blood glucose meter(s) if you
have diabetes. If you have multiple glucose meters or wear
an insulin pump and/or sensor, please arrive 15 - 20 minutes
early to allow us time to download the data held by these
devices before your appointment with your provider.
Parking is available on site at each of our
locations. Please allow extra time for traffic and making your
way to our office from parking facilities. Patients arriving
at our office more than 10 minutes late for an appointment may need to be rescheduled to a future date.
Regular appointments are important in providing quality care. If you are an established patient and must cancel an appointment, we ask that a minimum of
72 hours notice be given so we can assign the time to another patient.
We may charge a $25.00 fee for appointments missed without prior
notice to us.
Our phone hours are Monday - Friday 8:30am -
4:30pm, closed for lunch from 12:15pm - 1:30pm. For our
